Legacy/Legacy 2.0 Interest Form


Thank you for your interest in our Legacy program designed for first time parents of children birth to two years old, or our Legacy 2.0 program, designed for parents who have had another baby and would like to participate in the dedication service. We currently offer LEGACY dedications twice a year. Please fill out this interest form and we will notify you when upcoming dates are available.

Legacy is for first time parents and Legacy 2.0 is for parents of multiples
